This report identifies permits and documentation required to obtain a permit for removal or restoration of the building to a safe condition. This report is not an appraisal for financial claims. The building inspector may require portions of the building or other debris to be removed to allow evaluation of the damage. Structural shoring and additional lighting may also be required. Before demolition or restoration can begin, an approved method of trash containment and removal must be in place. DATE: 8/25/2026 ADDRESS: 530 S DOBSON RD 467 MESA DAMAGED CAUSED BY: Fire INSPECTED BY: Richard Lemcke The inspection of the site identified the following: Please note that before a damage permit is issued an estimate showing the Company doing the work, the subject address, scope of work, materials, and labor from either the contractor or the insurance company will be required. A written scope of work is required to approximate the cost of the permit, verbal estimations shall not be used. A completed Contractor Verification Form must also be submitted prior to being issued a permit. This structure is to be restored like for like construction in compliance with 2023 NEC, 2024 IRC/IBC requirements. Any alterations or changes will require submitting plans before any restoration can begin. During the damage inspection the following was observed: Structural members were damaged and will need replacing and/or repairing Electrical sustained damage to several home runs All damaged wires must be replaced per 2023 NEC/2024 IRC. Mechanical equipment was damaged, and all mechanical components will need replacing. Plumbing does not appear to be damaged, however it will need to be inspected during frame inspection Note: Any structural members charred to a depth of 1/8” or greater are to be replaced. All plumbing, electrical and mechanical systems damaged by heat are to be replaced. A. Building is: Commercial B.
